Checklist for Product Submittal for Safety Certification

Having the correct information and test samples ready helps to ensure a smooth safety-certification process. The information needed for submittal may vary slightly between agencies. The following list describes the items needed to obtain a quote:

  • marketing brochure or data sheet with photo describing the product (how and where it is used);
  • product model or series number and rating (voltage, current, operating temperature);
  • critical-components list with marks verified and alternate components listed;
  • schematic and/or wiring diagrams;
  • name, address, phone, and e-mail of the applicant, and contact name;
  • date the sample will be ready and the date completion is needed; and
  • statement of work: preliminary evaluation or assessment for certification (Mark).

Subsequently, you must submit the following items:

  • test samples;
  • safety-critical components certificates;
  • user documentation/manuals;
  • rating and caution labels, markings, or drawings;
  • 1 x 1 printed-wiring board artwork (outer layers) for noncertified CCAs >42.4 Vp/60 VDC;
  • factory address information and contact name;
  • completed application forms and factory inspection agreements;
  • purchase order and deposit, when requested; and
  • any other items as requested.
